it depends on your policy. for mine, once i met my $500 deductible (which is billed to you by whomever is doing the surgery) if it was an in-network provider it was covered 100%; out of network provider was covered at 80%. in-patient stay was a $150 charge on top of that and out-of-patient stay was subject to a $300 deductible. all in all i was pleasantly surprised, i didn't think it was going to cover ANYTHING---i can't afford the $5900 out of pocket cash cost for a reversal for but i can afford the $650 in-provider cost. I'm ecstatic. and the customer lady was REALLY nice, too.
From my research on https://provider.bcbsga.com/provider/medpolicy/ it seems as though reversal of the procedure is covered, however the actual procedure is not. However I highly suggest you contact BlueShield of Georgia to get more specific information for your coverage plan and to confirm this information. Hi I have BCBS coverage elsewhere, and my card has an 800 number on the back to customer service, and I've always found them reasonably useful. However, rather than trust the word of some 8-dollar an hour CS person, I would advise you to have your doctor's office prepare and submit the forms, and wait until your are denied by someone whose signature carries some authority before you give up. You may be pleasantly surprised. We have been. Phil
There are essentially no insurance companies that will cover tubal ligation reversal. Many clinics that perform the procedure offer payment plans that spread the cost of the quite expensive surgery.
I had a tubal ligation with Medicare as primary and Blue Cross Blue Shield secondary in 2008 thru Allina clinic in MN. I am very mad that it ended up costing me $1500 out of my pocket which to me makes no sense! Medicare paid $3000 and Blue Cross paid less than $100.

Most likely not as this is a voluntary and unnecessary procedure.
No, it will only cover it if it is considered "medically necessary".

No Blue Cross Blue Shield of Georgia does not cover going to weight loss doctors. It may however cover weight loss surgery.
It depends on the plan you are on. Call the benefits office number on the back of your insurance card and ask.

No, because it is an unnecessary and voluntary procedure ... much like facelifts and boob jobs.
Most doctor's offices and pharmacies will have the information.
Yes, as long as your plan covers prescriptions.
