He has stated emphatically there's absolutely nothing wrong with him, he lost nearly 30 pounds by staying away from heavy deserts, sugar and alcohol. He really is not ill and feels 100%.

Mcgee isn't really that skinny. He is normal size.

In Season 7, he says he's going to lose weight and when his co-worker, Anthony Dinozzo, questions him about his weight, he reveals he has lost 15 pounds, but in all the preivous series, he is "overweight".

Actually speaking, he is perfectly fine. He does not have any health issues. He has just lost some weight and that is why people are tempted to think that he is sick. The good news is that, he is healthy and just fine.
