Nobody is ever actually on the phone with him. Remember when Hershel comes in and Rick tells him someone called, Hershel picks the phone up to check if it works and it's dead. Rick has a mental breakdown and he imagines it all. The people who were supposedly talking to him were people that he knew before that had died, in order by call: Amy, Jim, Jacqui, and Lori. When they were talking about the "safe place" where no one is ever attacked they probably meant Heaven or the afterlife.
