RafaViz Media is currently releasing Season 1 of Vampire Knight first, with Volume 1 (Episodes 1-4) currently out, Volume 2 (Episodes 5-8) coming out September 28th, and Volume 3 (Episodes 9-13) coming out November 16th.
Because of this pattern, I'm assuming Viz will release Vampire Knight: Guilty's thirteen episodes in the same manner and probably in the months of January, March, and May of 2011.

There is no accurate answer as they have not released any information about it yet. Although, the fact that the ending of season 2 left viewers with many questions, this could lead to another season. It's also possible due to the popularity of Vampire Knight. Also, Vampire Knight Destiny is from the manga, not made up.
