It happened when Peter Pan first visited Neverland and Captain Hook and his pirates confronted him. It was Peter's first time sword fighting and he didn't know quite how to do it. However, he saw that Captain Hook held an arm out for balance as he fought. So Peter flew and cut off his hand as a way to defeat him without sword fighting.
