The term comes form the days when print was set a character at a time by hand.
The lead stamps for each character were set out in trays so that the typesetter could reach out and get the one he needed. The stamps were sorted into two rows or cases, one on top of the other. The stamps for the small letters were always set below the stamps for the capital letters. Thus the capital 'A' was in the 'upper case', above the box for the 'lower case' small 'a'.
