Grains are an apothacary system of measurement. This is not a brand of Tylenol but rather a measurement of how much drug is in the pill. A grain is approximately 60 milligrams. Grain V is the amount of Tylenol in each tab or caplet. The V is the Roman numberal for five. When you are using roman numerals if the numeral comes before the the unit (grain) it means subtraction. That is, if the V was placed in front of the word grain then it would mean one fifth of a 60 mgs or a tablet of approx 12 milligrams. If the V comes after the unit(grain) as in this case it means 5 grains of Tylenol or 300 milligrams of Tylenol.

No such thing as Tylenol 1..The first number on Tylenol will be a #3 which is then 1/4 grain of codeine and yes, illegal if not prescribed by a doctor...anything below a three has no codeine so there is no number system for them
Apothecary measurements used grains, which measured (1 grain = 65mg), for all other medications, excluding Aspirin which is the same as Tylenol. Therefore, 10 grains of Tylenol is 650mg. This turns out to be one maximum strength Tylenol pill. Nelson final year nursing student.
