They could just be dry. Put a little Vaseline or lotion on them everyday and see if it helps. If not get baby oil and put it on when you're in the shower.

probably puberty, they do strange things around that time, but I'm a guy and this is womans health, you'll probably be fine don't sweat it, next time you see a doctor ask them. guys sometimes get one hard nipple. itching might have something to do with hair growth.

Because when blood rushes to the nipple it sometime gets mini clots in it and causes an irration.

Dryness around and on the nipple, try applying Vaseline or lotion after shower to ease the discomfort. If its hurts when itching, then seen a doctor, could be some infection.
