The point of maximal impulse (PMI) is the location where the heart is closest to the surface of the body. As there is less distance and tissue to impede the transmission of movement from the heart, the perceived impulse is felt most strongly at this point.
In adults it is frequently found in the fifth intercostal space just medial to the left midclavicular line.
In infants and young children the PMI may be found just left of the midclavicular line and at either the 3rd or 4th intercostal space.
Identification of location of the PMI, assessment of its mechanical character, and auscultation of the area are often of diagnostic value in the physical examination of the cardiovascular system.
