The side of the pen where the clip or cap is located is typically considered the top side.
I would think that it would be called the pen cap or the cap of a pen. Top of the pen or pen top may also be used. But I don't really believe that the lid of a pen is the correct use of the word "lid" in the phrase.
To refill a top-secret UV pen, unscrew the cap from the pen to access the refill compartment. Insert a new refill into the pen until secure, making sure to avoid any leaks or spills. Once the refill is securely in place, screw the cap back onto the pen and test the pen to ensure that the refill is working properly.
The end of a pen with the tip or point is considered the top, as that is the end used for writing.
The spring needs to be inside, it's purpose is to to force the battery(ies) to meet the cap. It is not too difficult to insert batteries in it, even if the pen requires several flat batteries. You just have to insert a battery, one at a time, then keep your finger on the battery, to keep it from dropping. Then you slid another battery under your finger. You repeat this until all required batteries are in the pen. Keep holding your finger on top of the opening. Then comes the tricky part: you have to slid the cap from side on the battery, replacing your finger. If this does not work for you, you can use the blade of a knife to hold the batteries in, flat on top of the tube, and then let the cap slide in and the knife-blade slip out. Do notice, that all the flat batteries have to be oriented same way. Most commongly flat side towards the cap and the 'bump' towards the tip of the pen.

Alright, darling, first grab that pen and twist the top off. Then, use a pair of pliers to pull out the ink cartridge. Finally, unscrew the bottom of the pen to release the spring and any remaining pieces. Voilà, you've successfully disassembled a Bic pen!
