The patient chart contains all the medical information which includes daily notes by the nurse and doctor. It contains all the procedures done during the patients entire stay within the hospital. It keeps a record of the patients stay with the hospital. May serve as legal evidence also.

A patients chart will have details of the patient's condition and may have ongoing monitoring of their condition, like how blood pressure or temperature change over a period of time. Anything numeric like that could be put on a line chart in Excel. You would fill in the numbers, possibly with dates and times and then chart them by selecting them and starting the Chart Wizard. More general details, like notes on their condition would not be in a chart, but stored as text, so it could be in a separate document like in a word processor. As each patient is different, so will their charts.
