A patients chart will have details of the patient's condition and may have ongoing monitoring of their condition, like how blood pressure or temperature change over a period of time. Anything numeric like that could be put on a line chart in Excel. You would fill in the numbers, possibly with dates and times and then chart them by selecting them and starting the Chart Wizard. More general details, like notes on their condition would not be in a chart, but stored as text, so it could be in a separate document like in a word processor. As each patient is different, so will their charts.

A chart locator system keeps track of the location of all records in the health care facility. Due to various situations, the patients charts may not be shelved in the permanent file location. (example: when some one is readmitted, the admitting department would need to sign out the patients chart, records could be signed out for research or for patient follo-up care.
