in olden days, the claim interaction between payer and health care systems were done on paper. Now its been automated and started following a pattern for sending information about a paient to the payer using a standard format called ANSI 837. it contains patients personal information, level of service provided by hospital to him in terms of HIPAA/HCPCS codes.
