I don't think there is a clear solution to stopping discrimination. It can start with you, when you see it happening step in. You can's change evryone else and the way they do things but you can change yourself.

If I suspect that unfair discrimination, bullying/harassment/intimidation are taking place in my work place i would report it to a relevant manager. I would try to support colleagues, service users or carers who make a complaint of discrimination, bullying/harassment. I will try to cooperate with any measures introduced to promote equality and diversity in the workplace
