Because health care practitioners and doctors are human beings, and they form opinions about what the best care is for each diagnosis. Their opinions can be marginally effective or actually wrong. The standards are developed by concensus of experts in each field of care. They are based on clinical research and data/statistics on the effectiveness of treatments. These Standards are published with the hope that everyone in the field will do the same thing for patients. Because of the structure of the medical care professions in the US, the Standards are not enforceable. That is why institutions get surveyed by agencies to determine if the care provided is adequate.
