Toothpaste is only dangerous when swallowed. The pea-sized amount is recommended for small children as they may accidently swallow the toothpaste and the small amount would prevent anything bad happening to them. Otherwise, toothpaste that contains fluoride is vital for helping to strengthen your teeth.
The more dangerous is fluoridated water.

Not necessarily. Certainly, some alkalis are poisonous but many are not. Toothpaste is an alkali. So is milk of magnesia. Both of these substances are very safe and actually keep up healthy.
