depends on the country where you are studying.

first you enroll in med school, after that you can apply for a position as a resident in training for some more years, depending on the country.

When you finish your anesthesia studies, you can still do a fellowship in a certain area of anesthesia, such as chrinoc pain therapy, intensive care, cardiac anesthesia, pediatric anesthesia, and so on...

for example, my schedule: med school 7 years, anesthesia training 5 years, then fellowship 1-2 years in cardiac and cardio-pediatric anesthesia.

fellowships are an option, but often recommended to do if you want to apply for a job in a well known hospital where a lot of people apply for the same position.
