category 2:
able to deep breathe & cough freely = 2
dyspnea or limited breathing = 1
apnea = 0


category 3
BP plus or minus 20 mmHg of pre-anesthetic level = 2
BP plus or minus 20-50 mmHg of pre-anesthetic level = 1
BP plus or minus > 50 mmHg of pre-anesthetic level = 0


category 4
fully awake = 2
arousable on calling = 1
not responding = 0


category 5
90% SpO2 on room air = 2
needs O2 inhalation to keep SpO2 >90% = 1
SpO2 < 90% despite O2 supplementation = 0
