If you have the Dawnguard DLC, then you can just go along with the Dawnguard Quest-line and choose to become the updated form of Vampire. In the "vanilla game" (game without dlc), you must be given the "Sanguinare Vampiris" disease by a Vampire. This is possible by being struck by a vampire and is preventable if you drink a cure poison potion. Vampires won't often be found in cities unless you have Dawnguard DLC, otherwise... go to a vampire dungeon like Movarth's Lair (outside Morthal).

If you just have the disease and you aren't a vampire yet, you can use any Cure Disease Potion or any shrine to cure yourself before you are a vampire. Once you are a lower level vampire talk to a innkeeper or bartender about rumors. They will point out a man from Morthal who can help you.
