Bleach's author, Tite Kubo, has a habit of not officially killing off any of the main protagonists. However, many characters within the show's continuity are killed. Here they are, and be warned now, THERE ARE MASSIVE SPOILERS!

The 9th Espada, Aaroniero Arruruerie, is killed by Rukia Kuchiki

The 8th Espada, Szayel-Aporro Granz, is killed by Mayuri Kurotsuchi

The 7th Espada, Zommari Leroux, is killed by Byakuya Kuchiki

The 5th Espada, Nnoitra Jiruga, is killed by Kenpachi Zaraki

The 4th Espada, Ulquiorra Cifer, is killed by Ichigo Kurosaki

Actually, Kaname Tousen was nearly killed by Shuuhei Hisagi, but as Hisagi, Komamura, and Kaname were having a heart to heart, Aizen appeared on scene and killed Kaname. Kaname essentially blew up, as evidenced by the massive blood stain in the anime.

We know Aizen killed him because Komamura got really angry at Aizen and was about to go and attack him but then Ichico came through the Garaganta.
