I think it operates on several levels:

Man is depicted as imperfect (holding a cane), with baggage (holding a bag). Although he has a body (depicted as an open cage), it operates in two worlds - the two birds, one in, one out. This would be the physical and non-physical aspects of life. The one inside is where our heart is, and the bird looks in. The bird outside looks out, expressing how we extend into the world. I think it shows how any healing mankind does grows from our ability to bridge the gap between non-physical and physical. I say that because the background is a pretty seaside natural scene as opposed to his normal non-descript ones.
