What episode does sailor moon have a child?

In Sailor Moon R, episode 53 (47 in the English dub), Usagi and Ami walk by a nursery school and look at all the cute babies. Shortly thereafter, a cardian sent by Ail and An attacks, hoping to get the "fresh energy" from the babies. Sailors Moon and Mercury intervene, saving many of the babies, although some, as well as some of the parents and nursery school workers lose a great deal of their energy. The cardian disappears before they can defeat it, though, leaving Usagi and Mamoru to care for one of the babies, Manami, until his mother recovers.

In the Japanese episode 125, the third-to-last episode of the Sailor Moon S arc, Sailor Moon helps/rescues Sailor Saturn at the end of the episode, after Sailor Saturn enters Pharaoh 90 in an effort to destroy it. Sailor Moon refused to believe that Saturn would have to die to save everyone from Pharaoh 90, and went in after her--emerging from the battle with her Pure Heart crystal floating outside of her body and a baby Hotaru sleeping in her arms.

Yes, in fact, she does. Her daughter, Sailor Mini Moon, or ChibiUsa for the manga purists, is the dughter that comes from the future because she feels forgotten. In time, she explains that she is Momaru and Usagi's future child, who is guarded by Sailor Pluto.

They don't actually have a baby until later in the future. Their baby however does go back in time like Rini/Chibiusa does. Her name is Diana. She comes in episode 133 "Much Ado About Kitten" / "Artemis' Affair!? A Mysterious Kitten Appears" :)

Sailor Moon is never shown giving birth or even being visibly pregnant, but it is stated in the anime episode 83 (76 in the English dub) that Chibiusa is Usagi (Serena/Sailor Moon) and Mamoru (Darien/Tuxedo Mask)'s daughter.

In the manga, it's strongly implied that Usagi is pregnant prior to her wedding in the 30th century, after the Galaxy Cauldron has been reset. The opening scene of the very last few pages shows Usagi and Mamoru waking up in bed naked together.
