Well, technically there are 12 districts (numbered 1-12, go figure) and the Capitol. There is also District 13 that was thought to have been obliterated in the rebellion against the Capitol, though it was actually keeping alive, safe from the Capitol for their fear of District 13 retaliating with the nuclear bombs they manufacture.
District 13 was destroyed during the Dark Days, which ultimately caused the creation of the Hunger Games.
A district cannot win the Hunger Games, only tribute from one district can win each year. (Except the 74th annual Hunger Games)
In the second book and film, Catching Fire, District 12 is destroyed by an aerial raid by the Capital, but that is offscreen
