Sailormoon does actually die a grand total of 6 times.
1. She dies in the Silver Millennium as Princess Serenity. In the manga, she stabs herself. In the anime, she's killed by Queen Beryl.
2. She dies in the first season of the anime (This is in the subbed version). When she is fighting Metallia, she maxes out the power of the Silver Crystal. Maxing out it's power usually means death for the wielder, though one wish is granted before that person's death. Sailormoon wished for everything to return to how it had been before.
3. In the Sailormoon R Movie, she again, maxes out the power of the Silver Crystal. She is, again, revived.
4. In Sailormoon S (and this is debated), when Sailor Saturn performs the attack that kills everything, Sailormoon is killed, as is Sailor Saturn, and as are a lot of other people but they are all revived - Sailor Saturn as a baby, and Sailormoon in her regular form.
5. In Sailormoon SuperS, she has her dream mirror removed from her. It is then shattered, killing her.
6. In Sailormoon StarS, she dies to rather dramatic effect.

Galaxia doesn't really die. Sailor Moon manages to get chaos to leave her body and then she leaves earth to guide all the star seeds back to their original planets.
Technically, she dies in Season 5 (Sailor Stars) (which was never dubbed in English), but Sailor Moon defeats the big baddie, Sailor Galaxia, and all the people who died (including Chibimoon) come back to life, kind of like what happened in the climax of Season 1 (Sailor Moon Original/Classic).
