The Grim Reaper will come to take your sim to the afterlife, if you have another sim that has a high relationship you can click on the Grim and Plead For Loved One.If you win, the sim will be brought back to life, if you lose, your sim will be taken away by the Grim.
Different ghosts have different colours, depending on how they died, and every night they will haunt you and reenact the way they died over and over again.
If your sims get tired of being haunted, you can create a cemetery lot (community type), and have the sims click on the tombstone(s) or urn(s), and choose "move" and it will allow you to select that lot. Then, you can enter that in buy/build mode, and move them where you like.

This has never happened to me, but I think the baby dies along with the mother. You can plead for the mother and baby, or, with the career rewards, get the Grim Reaper phone and pay for them back, which I think she will still give birth

One of your sims has to die. After the sim dies, the Grim Reaper shows up and takes the sim with him. He leaves behind a tombstone or urn (depending on how the sim died). If you don't delete the tombstone or urn, then once in a while the sim's ghost will wander around the lot. It will only be on the lot where the sim died. You can't control the ghost, and it will scare people.
