It means that the server or the person connecting to the server do not have the same game version. If the server is v1.7.2 and the person connecting to the server only has v1.7.0 then they can't talk to each other. Or it can be the other way around. They both have to have the same version of the game.

An outdated client simply means that your current version of minecraft isn't up to date with Mojang. It's okay to have a outdated client simply because some minecraft servers take longer than others to update with Mojang, that's why all versions of minecraft are available through the minecraft launcher. Some mods require older versions of minecraft as well. The newest version of Minecraft PC that is out right now is 1.10.2. One of the servers I play on is still using 1.9.4 because they have to update their code to support 1.10.2, so I run a 1.9.4 client.
