To get off Telos you have to first choose a faction to help, either the Ithorians or Czerka, the Ithorians being the light side choice and Czerka being dark. Play through the quests you are given by either faction.
After completing the quests for either faction they will then provide you with a shuttle which will take you to the surface of Telos. You will then meet an Iridonian named Bao Dur who will help you to find the Ebon Hawk. To find it you have to fight through an old military base where you will find a second shuttle which will take you to the polar regions of Telos.
Here you will find a hidden Jedi Academy and one of your old Jedi masters, Atris. After a brief talk with her she will tell you that she stole the Ebon Hawk from Citadel Station and she will let you have it back. You can then fly away from Telos. (:
