If you 'Trap Hole' a monster whose effect activates when it is flip summoned is the effect still activated but the monster destroyed?

The effect activates first, and you are actually chaining Trap Hole to that trigger. Trap Hole will resolve first, but it will not stop the monster effect from resolving. Unless a card says it negates a monster's effect, then it is still able to resolve its affect when flip summoned.

Let's say though that he survives destruction, perhaps he had a Card Guard token on him. If that happens, then he will trigger after Dark Hole resolves, summoning the monster. Since Dark Hole has already resolved, the newly summoned monster is in no danger from it.

If the above monster was simply discarded from hand by something else, then it can not be special summoned by Monster Reborn. If it says 'cannot be special summoned except by..' then it can't be brought back with Monster Reborn even if it was properly special summoned first.

Equip cards can never remain face-up on the field without their target monster. If the monster leaves the field or is turned face down, then the Equip card will be destroyed. That goes for regular Equip Spell cards, but also monsters stolen and turned into Equips by Relinquished, etc. However keep in mind that cards like Call of the Haunted do not actually 'equip', so can remain on the field under certain circumstances, like if the summoned monster was tributed instead of destroyed.

MB-EWD will simply be destroyed if it is ever face-up on the field, and no Field Spell Card is in play. So if Monster Reborn was used in this case, MB-EWD would be special summoned, and then destroy itself.

When a non-effect monster destroys another monster the destroyed monster is sent to the grave, unless there is another card on the field which changes where destroyed monsters go. Depending on the effect of the monster that destroyed them they are either sent to the Graveyard, are banished, return to the owners hand, or can be summoned on the opponents side of the field. Some cards can even be saved from the grave by their own effect and are then either re-summoned or go to spell/trap zone. Spell/trap cards can also determine where they go, as they can change it so that any destroyed monster is banished instead of being sent to the graveyard.
