For Mac and Windows, $10 gets you the whole game: Episode 2: Slough Creek, multiplayer, a WQ account key, Lost River, all coats and customizations. For mobile devices, in-app purchases can be purchased separately for $3.99 -- this gets you multiplayer, a WQ account key, Episode 2: Slough Creek and Lost River, but none of the new wolf coat packs or body customizations.
There is no recurring (weekly, monthly or yearly) charge. Buying the game is a one-off transaction unless your account ends up banned or disabled by the WolfQuest Team for rule-breaking. You would have to buy the game again to receive a new account key if this happens.
Edit: I would like to add that at the time this answer was provided (June 2017) this was valid. In late 2017 and perhaps from 2018 onwards, this has the potential to change.

Contrary to popular belief, you do not need to complete WolfQuest to unlock multiplayer. The feature is available immediately after installing the game-- to play, you will need to have registered an account on the community forums in order to login and play with other members of WolfQuest.
