According to one walkthrough I found, if you go to the gardener and ask her if you can help, she'll ask for a cure for her plants, then she will give you the spade in return. Then go to the doctor, and ask her for help, and she'll tell you what she needs to make the cure. Gather those items, then take the cure back to the gardener. I've done that in two different games and while the gardener is thrilled with the cure, she doesn't give me the spade. become friends with her and she will ask you to do something else for her once you complete that she will give you the spade
