You need to first have a married couple ready that want to have a baby. Then you buy a crib for the baby. When you put it in the house, you click the icon above it that says add a baby. This will cost lifestyle points and you will have to wait. But you can always skip ahead the time on your device. From there, you can pick out the baby's clothes, eye color, hair, name, and skin tone.
