There is a 10% chance of your sims having twins but if you really want them to then do Ctrl + Shift + C and a cheat box will pop up at the top of your screen and then type in "ForceTwins". If that doesn't work then you have to type in the cheat box "boolprop testingcheatsenabled true" then select the pregnant sim then Shift + Click on that sim and go to Spawn>Tombstone of L and D and a tombstone will pop up next to your sim click on that and it will say something like "DEBUG - ForceTwins" and click on that and then your sims will be pregnant with twins. The only thing is that it is random of if you get twins girls, boys, or one of each. I hope that helped.
