You must have the title of knight or above in order to conquer a city. Knight lets you conquer one city. Each additional increase in title entitles you to one more city. You cannot conquer another player's city UNLESS that player has more than one city.To conquer another player's city you should first scout that city to see what defenses and army it has. Then you need to send an army big enough to destroy the defenses. Then you need to send wave after wave of additional troops to reduce the city's loyalty to zero. At first, each attack will reduce loyalty by 3 or 4. This declines until below 10 each attack reduces loyalty by only one. WARNING: If the gates are closed, they will automatically open when loyalty gets down to 5 and you will have to fight their entire army. Also note that once you get their loyalty down to 1, you have to wait 6 minutes before you can get it down to zero. At zero the city becomes yours.
To conquer an NPC there are a few differences. First, their gates are always open. Your first attack fights their entire army. Second, their army continually rebuilds so even if you destroy their entire army and all defenses, every one of your attacks will still have someone to fight. Defenses and army rebuild at the rate of 1/60 per minutes. If your waves are too far apart you will have high losses and may never win. Always have plenty of reserves. Also, NPC loyalty is restored by 1 every 6 minutes so don't wait around. Last, there is no 6 minute wait to get it down to zero.
