If you want to make a makeover, just click on mirror - Change Appearance. If you want to make a plastic surgery, you'll need a University EP. Graduate from university and get a job in Showbiz. Work hard and then you'll receive a career reward - Dr. Vu's Automated Cosmetic Surgeon. Be sure you have at least golden aspiration level to avoid failure (if it happens just try again). OR you can also use cheat. Press ctrl+shift+c and type "unlockcareerrewards", then choose Dr. Vu's Automated Cosmetic Surgeon and locate somewhere on a lot. Use and enjoy.
