Non of the optional Dual Shock 3 color varieties are rare because they are mass produced. Unique controllers sold with certain games are also not that rare because they are also mass produced. Your best chance to find the lowest number of controllers might be in some the the very expensive driving wheel systems that are unique controllers that will not function on every game or maybe a High Priced limited edition optional Guitar Hero type controller. Even these controllers would be mass produced and are likely to still be available new on amazon or other sites.

It is a code that is unique to each individual Hardened addition game and not a code for all the games. It is not able to be reused except maybe on the original Xbox 360 or PS3 that it was entered. Used hardened edition games no longer have the 4 extra maps

Think of a code as a key code like when you purchase software or the individual code each Playstation Network card uses. Besides the 4 WAW zombie maps are available along with the Moon Map with the Rezurrection map pack for $15 for the systems that had hardened and Prestige edition versions of Call of Duty Black Ops. It was never a feature of the hardened and prestige editions to unlock all the zombie maps either and they only got the Black Ops version of the 4 WAW zombie maps and still had to purchase First Strike, Annihilation, & Escalation DLCs
