In the manga he calls her "ugly pig". Not everyone's gonna think someone is beautiful, and the Sakura stans insisting he didn't mean it are just in denial. Sakura notoriously had a huge forehead and flat chested, so for someone to objectively think she's ugly isn't crazy.
