Why did itachi cry when he left Sasuke?

Yes because Madara captures him after and tells him that all Itachi had done was for him. That he had planned it all along to protect Sasuke, and that the Uchiha Clan Massacre was not Itachi's plan but the Village's plan, to protect Konoha from the clan.

But Itachi said something to Sasuke before he died.

He said he always love him as a little brother.

Then out of Sasuke's eye blood started dripping,

so that might be tears and blood mixed thus he might of cried.

This answer contains information of the Manga] Itachi does indeed cry, Sasuke realises this when talking to Madara Uchiha and when we do find out; it was actually the night Itachi killed the Uchiha Clan. Itachi also cried again when standing in the rain; (Not Confirmed, but Kisame said "I could actually say you were crying") the time of Sasuke's revenge was near. Though Itachi is not like a frequent crier that we may/or may not know of; but these two dates are the only evidence recorded of Itachi crying.

Upon realizing the truth, Sasuke regrets killing Itachi thinking back to when he was a kind, loving brother and always was. He formed his Mangekyo Sharingan after losing Itachi, the one closest to him. He vows to destroy Konoha (Hidden Leaf Village) for putting Itachi at the fate he had. He later joins the Akatsuki and becomes an international criminal, something that Itachi tried to prevent.

He loves his baby brother. Even though he dosen't show it much, there are a few times in the show where you can spot it.For example, in episodes 84 and 85, (Regular Naruto), Itachi murders the whole Uchiha clan because he was ordered too, but he dosen't kill Sasuke because he loves him too much. You don't find this out until episode 140-143 in Naruto Shippuden...where the reason why Itachi killed the Uchiha clan is explained.

Anyway, when Sasuke and Itachi are fighting in Naruto Shippuden, ( episode 135-139), you'll finally get to see some brotherly love from Itachi. Itachi has always loved Sasuke, but he just never tells him until Sasuke is killing him....which is quite sad.
