Shelby has kidney disease. But she desperately wants children but fears her condition will prevent her from carrying a child. While the women are having their hair done, Shelby has an episode of low blood sugar. Here is part of the dialogue from that scene as Shelby is recovering from the low blood sugar incident, as they talk about her desire to have a child:
