This was basically to save introducing and building up a new character in the movie. There was a lot of storyline to fit into the movie, so they saved time by changing the snitch to Cho, much to the demise of many book fans.
in the 5th movie or book the order of the phenix
In the book she didn't. It was Marietta Edgecombe. In the movie however it was Cho. At one point Harry tells Cho that he heard Umbridge gave her a hard way to go. Later, when Harry is caught by Umbridge and the Inquisitorial Squad, Umbridge calls Snape into her office. She demands that he bring her more Veritaserum (a truth serum). Snape advises her that she used the last of the serum on Miss Chang.
Her patronus is a swan.
There Is no evidence from the book to tell us, but it may have been Cho Chang.
Cho Cho Cheng was born on December 29, 1985.

Harry has kissed two people during the series, Cho (in the fifth book/movie) and Ginny (in the sixth book/movie and afterwards).
Ginny Weasley and Cho Chang are not places, so I'm guessing your asking who are Ginny and Cho. Ginny Weasley is Ron Weasley's younger sister. She is the youngest Weasley and the only girl. In the epilog of the last book/movie, she marries Harry Potter. Cho Chang first appears in the fourth movie, Harry Potter and the Goblet of Fire. She is Harry's first love interest, but is dating Cedric Diggory through out the movie/book. When Cedric is killed at the end of the fourth movie/book, Harry briefly dates her. After the fifth book/movie, she is no longer an important part of the series.
Yes, in the book. In the movie, she was cut and Cho did it under the effects of Veritaserum. In the book, however, Marietta betrayed the D.A. of her own free will.
In the movie, yes (but only under the effect of Veritaserum). In the book it was her friend Marietta Edgecombe.
The dog character in the movie is called Cho Cho, but it is played by a dog named Rex.
No because Harry already got a partner in the 6th book and Cho already went to the Yule ball with Cedric Diggory.
No, they don't kiss in the book either. They are embarassed around each other, and don't even talk. Harry kissed her in the Order of the Pheonix, but he kisses Ginny in the Half Blood Prince.
Cho Chang. In the book it is her friend, Marietta Edgecombe that reveals the hiding place.
