Deidera of the Akatsuki killed him when he extracted the Shukaku demon from him, though Chiyo (the Sand kunoichi who sealed the demon in him originally) sacrifices her life by trading her life for his. He lives on and protects the leaf ninja as well as his brother, Kankuro, and sister, Temari. He does retain his ability to manipulate sand, though his powers are severely diminished, but they will return as true strength is found by protecting loved ones and family.

-he dies in the first season of naruto shippuden. dieradara (from the akatsuki. he's the blond artist guy) knocked gaara out. then he goes to one ofthe akatsuki hideouts and traps gaara into the big weird machine that takes jinchuriki's power. they took gaara's shukaku and it kills him (it sucks cause he's not special any more). but eventually at the end of the season chiayo uses the reanimation jutsu to bring gaara back to life but chiayo dies. even though gaara doesnt have his shukaku, he can still use his sand jutsus and everything he had before
If you are talking about back in their younger days before they were known and the Legendary Sannins and before Orochimaru went rogue then that would be Hanzo of the Village hidden in the rain. If you are talking about Orichimaru being defeated. Sasuke defeated him and absorbed him and then at the end of his fight with his brother Itachi, sasuke ran out of Chakra and Orochimaru was able to escape from within Sasuke. But Itachi then Sealed him using the legendary sword of Totsuka, traps him in an inescapable dream-like realm, robbing Orochimaru of his chance to gain the Sharingan. Jiraya was defeated and killed by Pain who just so happened to be his former pupil while he was attempting to gain information on the Akatsuki. Tsunade was drained of her Chakra while defending the Leaf Village from Pain and was rendered to her true age.

No, gaara is not killed by deidara. They had a brief battle, but gaara was merely trying to defend the village, and not really attack, hoping he would flee. When gaara noticed that deidara was trying to capture gaara, and gaara alone, instead of going all out and killing deidara, as he easily couldve, he defends the village from his ultimate,and willingly lets himself be captured. You see, since gaara killed so many people when he was young and was filled with so much hatred, now, he cares about the village, and since he is the kazekage, he cares even more, hell gladly let them take his life in spare of his own. He gets killed when the extract the one tails from him, but is ultimately revived, as chiyo sacrifices herself for him. :3

Yes, Gaara has no last name. Neither does Kankuro or Temari, seeing as they are brothers and sister. I do not believe TenTen has a last name either.
