Ian McDiarmid portrays the Emperor in all of the Star Wars movies (including the prequel trilogy) now with the creation of the new version of the original trilogy. Originally, the holographic image of the Emperor in The Empire Strikes Back the Emperor was the composite of an unnamed actress with the eyes of a chimpanzee and voiced by Clive Revill.
His in-universe name is Palpatine (this may be a last name; if he has a first name as well, I don't know what it is).
