Originally, the holographic image of the Emperor in The Empire Strikes Back the Emperor was the composite of an unnamed actress with the eyes of a chimpanzee and voiced by Clive Revill.

His in-universe name is Palpatine (this may be a last name; if he has a first name as well, I don't know what it is).

In the movies he was played by Ian McDiarmid (except in The Empire Strikes back, where he was voiced by Clive Revill and portrayed by a combination of an uncredited older woman with eyes replaced by those of a chimpanzee in post-production).
