If you mean class form then: Hogwarts does not have these. It is simply houses and year groups. Harry is in Gryffindor with the rest of those in his house and same year. There is around 10 of them: Harry Potter, Ron Wealsey, Neville Longbottom, Dean Thomas, Seamus Finnigan, Hermione Granger, Lavendar Brown, Parvati Patil (and Padma Patil in the movies but not books).

From what I remember you have to pour that potion Parvarti and Padma made up into it. You stand on a little platform to reach them. I think you have to levitate them with Wingardium Leviosa though. PS although they say several times that you'll get caught, you won't so don't listen to them.
