What they are doing is coming up with the music on the spot. It takes a lot of talent to do this. But, you have to know simply, what key the band is playing in and improvise...
Improvisations are unscripted acting scenes You are usually given a situation, such as you and your boyfriend are in a coffee shop, when his ex girlfriend enters with a baby, lays it on the table, tells him it's his, and he can have it.........now go. Then the two actors would act out a scene off the top of their heads, nobody knowing where it's actually going to go.
Improvised playing is related to but not identical with playing 'by ear'. In the very broadest sense there are generally two ways to approach a musical instrument. You can learn to read music from written or printed scores, or you can just jump in and start making music without the benefit of any sheet music at all. A person who can play 'by ear' is good at reproducing on an instrument a tune she/he has heard. People who play by ear can become very accomplished and can have a wide and interesting repertoire. There is a story the truth of which I cannot attest to that the great song writer Irving Berlin couldn't read music particularly well. He had a "transposing" acoustic piano, physically manipulated, that allowed him to 'play' in various keys while really playing the keyboard only in the key of C.
Improvisation adds the element of novelty, and a person who plays 'by ear' may do much more than reproduce familiar tunes. Jazz musicians can think and interact with one another using harmony as the framework of a number, and can explore their instruments very freely and creatively. An expert in improvisation can take her/his instrument and do things, essentially on the spot, that are novel and very satisfying. Improvisation can be completely free from any restraints, or it might be guided by underlying harmony 'changes' that a group of musicians is following.
Most think that this kind of playing has its origins in Jazz and the kinds of music that immediately preceded it. But as far back as Bach, a lot of music was written in a very technical form called 'Figured Bass', where various notes, symbols and notations allowed a skilled player to fill in the gaps in very much the same way that skilled Jazz musicians do.

Assuming that "real" music is defined as music derived from one's soul, then yes, it is possible to find music fueled by emotion. However, one must be able to see past what entertainment corporations push as "music." A good example of real music is improvisational music.
