Yes, Dumbledore is killed by Snape in Harry Potter and the Half-Blood Prince.
Voldemort made seven Horcruxes. Harry destroyed one in his second year and Dumbledore destroyed one just before his sixth year which left five Horcruxes to destroy (although he only thought there was four). In the end, every Horcrux was destroyed by different people (Harry, Dumbledore, Ron, Hermione, Crabbe, Voldemort, Neville)
This is a very broad question, and not worded correctly. Severus Snape killed Albus Dumbledore in the end of the sixth book. Dumbledore had asked Snape to kill him, to save Draco Malfoy's soul because of the Unbreakable Vow that Narcissa Malfoy made Snape take in the beginning of the sixth book since Voldemort had ordered Draco to kill Dumbledore.
Albus Percival Wulfric Brian Dumbledore dies in Harry Potter and the Half Blood Prince, the sixth book in the series, after being murdered by Professor Severus Snape on the highest Astronomy Tower.
When Harry "died," he actually was just dreaming. When Voldemort tried to kill him, he destroyed the horcrux that was located in Harry. The small child on the floor at King's Cross was the horcrux for Voldemort. Harry got up and went on to defeat Voldemort by using Malfoy's wand against Voldemort. The Elder wand's rightful owner was Malfoy, so when Harry used Malfoys wand against what was his own wand, the spells backfired. Dumbledore was killed by Snape in the Sixth book. Harry didn't die because of the horcrux.
Neville Longbottom destroys the sixth Horcrux, Nagini, Voldemort's snake, with the Sword of Gryffindor during the Battle of Hogwarts.

Yes, Dumbledore is killed by Snape in Harry Potter and the Half-Blood Prince.
Harry Potter is the horcrux Voldemort never ment to make. Voldemort made 5, then when he tried to kill Harry when Harry was a baby, a part Voldemort attached to the only living left in the room; Harry Potter. So, yes, Harry Potter is the penultimate (sixth) horcrux. The last one (the seventh) was Nagini.
There are seven horcruxes: Tom Riddles diary, Marvolo Gaunt's ring, Helga Hufflepuff's cup, Rowena Ravenclaw's diadem, Salazar Slytherin's locket, Nagini (Voldemort's pet snake) and the part of Voldemort's soul that is in Harry Potter. Then there is the eighth part of soul that resides in Voldemort's body (this part of soul does not count as a horcrux). However, Nagini was made a horcrux after Tom Riddle's Diary was destroyed so only six horcruxes ever existed at once (Nagini was made a horcrux with the murder of Bertha Jorkins in the sixth book).
warning spoiler if you haven't read the 7 book Snape is always a double agent he pretends to be working with lord voldemort as a spy to spy on dumbledore but in reality he is working with dumbledore to protect harry.

Dumbledore put a cursed ring on his hand which would have killed him had Snape not stopped it. Snape told Dumbledore he only had about a year left to live and Dumbledore knew that Voldemort was plotting his death but didn't want Draco Malfoy's soul to be damaged by this. He also knew that Voldemort would want the Elder wand and that Snape needed to keep convincing Voldemort he was on his side. So, Dumbledore told Snape to kill him which meant Voldemort trusted him, Draco kept his soul intact and Dumbledore died as the last true owner of the Elder wand - although because Draco disarmed him before he died that part of the plan didn't work. Also because Dumbledore says that he would rather be killed by Snape than one of the other Death-Eaters, such as Belatrix Lestrange who, as Dumbledore put it 'like to play with their food'. Snape also made an unbreakable vow to Narcissa (or Cissy) Malfoy, to help Draco in his deed set by 'The Dark Lord', and if it should seem Draco would fail, he commit the deed himself. (The deed being to kill Albus Dumbledore)
