This is called "improvisation". Generally, musicians improvise on stage, though it could get recorded. If a jazz guitarist improvises, he or she would be playing over chord progressions, scales, or "riffs". Improvisation happens on a constant basis in all styles of music, even in rock. Think of The Beatles' "Hey Jude". It's the same chord progression throughout, but the singing is 100% improvised. Improvisation is one of the most exciting aspects of music, for both the audience and the performers!
