Yes, he is in love with her. In her absence, Cesare is an exceptionally dark character who does a lot of very bad things. It is when he is with Lucrezia that we see his humanity and his capacity for goodness. She brings out the best in him, and is really the only person in the world capable of making him drop his guard.

He kills for her on more than one occasion, puts her welfare before his own and thinks of her before anyone else in a crisis. It is clear from the very early episodes that there is no one more important in the world to him.

In a more recent episode ****spoilers**** he kids to her about marrying her, running away, changing their names, and settling in an obscure fishing village where they would live out the rest of their lives in peace.

It was an interesting scene. Most interesting because Cesare never jokes about anything... ever. On some level, though he can never truly say it or express it, I think that's exactly what he wants.
