To quote directly from the book... The littlun Percival had early crawled into a shelter and stayed there for two days, talking, singing, and crying, till they thought him batty and were faintly amused. Ever since then he had been peaked, red-eyed, and miserable, a litttlun who played little and cried often. Percival had obviously been told that should he ever become lost he should recite his name, address and telephone number an that would enable him to be returned to his home quickly. Clearly marooned on a tropical island his incantation had failed to get him returned home. As a result he seems to have had a breakdown. Later, in chapter 5: Beast from Water when he is asked to speak during the meeting the other boys laughter caused him to turn to tears, no doubt reminding him of his earlier breakdown which the other boys had found 'fainly amusing.'

The main characters of Lord of the Flies are Ralph, Simon, Jack, The Hunters, Piggy, and Samneric. The other characters are The littleluns, Maurice, Robert, Percival Wemys Madison, Johnny, and Henry.
In my personal opinion, Percival shows to illustrate the loss of civilization on the island. Where in Chapter 5, Percival recites his name and address constantly, at the end, he cannot remember his own name. Coupled with Ralph's forgetfulness of why he was lighting the fire, the memory of civilization is clearly illustrated as fading.
In the last chapter of Lord of the Flies, a French officer asks Percival his name, but Percival can't remember it.
Percival
The boy who cannot remember his name in Chapter 12 of "Lord of the Flies" is Percival Wemys Madison. He struggles to recall his name when the other boys question him.

"Gib" is a shortened form of Piggy's real name, which is revealed to be "Percival Wemys Madison" in the novel "Lord of the Flies" by William Golding. Piggy is a character known for his intellect and glasses, which become a symbol of civilization and order on the island.
