The centre medal is for the Kalidor Crescent. I believe the only way you can get this is by accumulating enough bonus points (You CANNOT earn these if you have "Tour Collisions: Off"; "Tour Invulnerability: On"; or "Tour Infinite Ammo: On" selected in your General Options).
Not sure on the totals but I remember reading somewhere that it is 3500 bonus points per level and 21'000 bonus points for the full medal with all the trimmings as there are 6 levels of the medal which are: The Medal itself; Bronze Cluster; Silver Talons; Silver Scimitar; Golden Wings; Diamond Eyes.
