Here are how the Horcruxes are destroyed:
1. Harry stabs Tom Riddle's diary with a Basilisk fang.
2. Dumbledore stabs Marvolo's ring with the sword of Gryffindor, which is coated in Basilisk venom.
3. Ron stabs Slytherin's locket with the sword of Gryffindor.
4. Hufflepuff's cup is stabbed with a Basilisk fang (by Hermione Granger).
5. Ravenclaw's lost diadem is burned with magic fire caused by Crabbe.
6. Harry, the accidental Horcrux, was hit by the killing curse (courtesy of Voldemort) in Deathly Hallows and he survived yet again. However, the piece of Voldemort's soul residing inside his body was destroyed.
7. Neville cuts of Voldemort's snake's head with the sword of Gryffindor.
And finally Voldemort was killed by a rebounding killing curse that he used on Harry. It rebounded because Harry was the true master of the Elder Wand, and Voldemort was trying to use it on Harry. However, the wand knew it's master and would not curse him, so the spell rebounded.

Technically, no. There were two Horcruxes that Harry Potter didn't find: the Gaunt ring and himself. He also only destroyed one Horcrux when he stabbed the diary in his second year (although at the time he had no idea what he'd done). Albus Dumbledore destroyed the Horcrux that was in the Gaunt family ring before Harry even knew what a Horcrux was. Dumbledore never told Harry that he was a Horcrux until after it has been destroyed, he only (through memories) said that Harry must die to destroy Voldemort. The locket, cup and diadem were all found with Harry's help who also guessed that Nagini was the last one - but they were destroyed by Ron, Hermione, Crabbe and Neville.
Yes, Lord Voldemort did die in the end of the seventh Harry Potter book. When Voldemort killed Harry, he killed the part of himself inside of him as Harry was one of the Horcruxes. When Harry was carried back to the castle, Neville killed Nagini (Voldemort's Snake) which destroyed the last Horcrux. Harry then had a little dramatic scene where he and Voldemort are shooting one long strand of magic at each other and Harry overpowers Voldemort and kills him.

Through murder, the book says they can split their soul and put one of the pieces in a vessel or container. This stops them from dying, because even if they are killed, part of their soul is still kept safe in the horcrux. However, the soul depends on the container it is stored in, and it cannot survive if the container is destroyed. It is very difficult to destroy a horcrux, with methods shown in the book including basilisk venom or fiendfyre. It is also possible for the maker of the horcrux to make their soul whole again through remorse. The topic of horcruxes is said to be very taboo in the series, with most books mentioning horcruxes removed from the Hogwarts library. In the book, it was said that Voldemort created seven: his grandfather's ring, his diary, Helga Hufflepuff's Cup, Salazar Slytherin's locket, Rowena Ravenclaw's diadem, his snake Nagini, and (accidentally) Harry Potter himself. For each horcrux, a different murder had to be committed. Although Voldemort made seven, this was shown to be unprecedented, as it is very rare for hrocruxes to be made at all, and when they are, usually only one is made.
Harry, Ron and Hermione track down the remaining Horcruxes which involves stealing one from Bellatrix Lestrange Gringotts' vault. This alerts Voldemort who decides to start checking each Horcrux is safe. Harry sees this and discovers the last Horcrux (aside from Nagini and Harry whom they don't know about) is at Hogwarts. A large battle the ensues and all of the plot lines are tied up allowing Harry and Voldemort to come to their final battle.
