The story she told was that when she was shot by Stewie and went overboard, a merman came and saved her underwater and brought her to shore, and she found her way back to Quahog from there.
- Also it should be noted that the entire scenario was just a simulation so she didn't really get shot so how she 'survived' is a moot point.
Of course, if the simulation was to test Stewie to see if he could kill Lois and take over the world, one could question why he would program it for her to survive but that's putting too much logic into a cartoon.
