When Peeta saves Katniss, it affects her because she feels like she owes him more than he owes her (if that makes sense); Katniss feels that he has done more for her than she can ever make up to him. Also, he loves her and she tries to push it away because she likes Gale.

So all in all, this affects Katniss because it hurts her conscience and her love life.

Because since Peeta really was in love with Katniss and that the "star-crossed lovers" wasn't a game at all to him, he would sacrifice himself for her, which almost happened since once Cato found out why Peeta really joined the Career Tributes, he stabbed him in the leg with a sword and if Katniss hadn't found Peeta when she did, he might have died, and he almost died from infection, for the wound was bone deep

Katniss receives sleeping syrup in a sponsor gift. She adds the syrup to a broth she gives Peeta and once he has fallen asleep from the effects of the syrup, Katniss easily slips away and goes to the feast.
